Question types
The question types you can add to a form, including short and long text, choice fields, rating, NPS, and CSAT.
Question types
Add questions from the field palette in the Build tab. Every question has a label, optional help text, and a Required toggle. Some types add their own settings, described below.
Text
- Short text — a single-line answer for things like a name or a brief title. You can set a minimum and maximum length.
- Long text — a multi-line answer for open-ended feedback or details. You can set a minimum and maximum length.
Both support an optional placeholder shown inside the field.
Choice
- Single choice — pick exactly one option from a list shown as radio buttons.
- Multiple choice — pick any number of options from a list of checkboxes.
- Dropdown — pick one option from a dropdown menu, useful when there are many options.
For each of these you add the options respondents choose from, and you can reorder or remove them.
Rating
A rating question collects a score. You can configure:
- Scale — 1 to 5, or 0 to 10.
- Preset — leave it as a plain rating, or choose a preset:
- NPS — a fixed 0 to 10 recommendation scale, scored as Net Promoter Score in results.
- CSAT — a 1 to 5 satisfaction scale, scored as a CSAT average in results.
When you choose the NPS or CSAT preset, the scale is set for you. See Reading results for how each is scored.
Asks for an email address and checks that it looks valid. When the form is set to ask everyone for an email, this is how a response gets linked to a customer — see Who can respond.
Number
Collects a numeric answer. You can set a minimum and maximum value. Number answers are summarized with an average and a distribution in results.
Date
Collects a date.
Yes / No
A simple two-option question. Like choice and rating questions, a Yes / No answer can drive conditional logic for later questions.
Display blocks
Display blocks show content without asking for an answer:
- Image
- Audio
- Video
Use them to add context, instructions, or a media clip between questions. They don't collect a response.
Required questions
Toggle Required on any question to make sure respondents answer it before they can submit.
