Create a form

Start from a template or a blank form, add questions, and use the builder tabs to configure, style, and review your form.

Open your project, go to Forms, and choose New form.

Start from a template or blank

When you create a form you can pick a ready-made template or start from scratch:

  • Blank form — an empty form you build question by question.
  • Templates — pre-filled starting points grouped by what you're trying to do:
    • MeasureNet Promoter Score (a 0–10 recommendation question plus a reason) and Customer Satisfaction (a quick 1–5 rating plus a comment).
    • CollectFeature Request (a title, area, and details) and Lead Capture (name, email, company, and interest).
    • RetainCancellation Survey (a reason, an open comment, and an email).

A template just gives you a head start — every question is yours to edit, reorder, or remove.

A new form starts as a draft. It won't accept responses until you publish it, so you can build and preview without anyone seeing it.

The builder tabs

The form builder is organized into four tabs. A live preview sits alongside the editing tabs so you can see your form take shape as you work.

Build

Add and arrange your questions. Choose a question type from the field palette to add it, then click a question to edit it — its label, help text, whether it's required, and any type-specific options. Drag questions to reorder them, and duplicate or remove any question.

You can also add display blocks (an image, audio, or video) that show content to the respondent without asking for an answer.

Configuration

Decide how the form behaves:

  • Map answers to feedback — choose how each answer is stored on the feedback it creates (for example, send a long-text answer to the message, a rating to the rating, or a choice to the category). Sensible mappings are suggested for you.
  • Who can respond — anonymous, email, or signed-in. See Who can respond.
  • Context capture — what page and device details to record with each response.
  • Distribution — the hosted link, QR code, website embed, and email button. See Sharing your form.

Appearance

Style the form to match your brand:

  • Brand — accent color, logo, and an optional cover image.
  • Style — button style, corner style, and font.
  • Layout — show one question per page or all questions at once, and toggle the progress bar.
  • Screens — optional welcome and thank-you screens, each with its own heading, body, and background.
  • Branding — show or hide the "Powered by UserHero" mark (removing it is available on a higher plan).

Results

Once responses start coming in, this tab shows per-question summaries, NPS and CSAT scores, and a CSV export. See Reading results.

Test before you publish

Use the test link in the Build tab to open the live form in a new tab and try it before publishing — submit a few answers and check the flow end to end.

Publish

When you're happy, publish the form to start accepting responses. You can close a form later to stop new responses, then publish again to reopen it.
